1. Open up a new document. I'll be making mine 378 x 35
2. On your layer palette, right click on where it says background and click Promote Background Layer. Hit the delete key once.
3. Your document should now be transparent. Go to your text tool, ( the 'A' tool. ) and type whatever you want. Mine looks like this;

4. Now, go to Effects > 3D Effects > Inner Bevel. A window will then pop up. Match your settings to mine:

5. Now click ok, and you're finished. Your text should look something like this:
